Smith Chiropractic Offices P.C. believes in the value of wellness care for you and your family and have maintained a pricing structure that allows care for all budgets. Our office participates in most insurance programs. We also accept auto accident and workers compensation cases. For patients who have little or no chiropractic insurance coverage, a reduced fee schedule is an option.
Insurance
Your insurance policy is a contract between you and your insurance company. We are not a party to that contract. Please remember that most medical insurance is not designed to pay the true value of the services and may provide for only a portion of the fee. When we or if you call to verify your insurance coverage this is not a guarantee of payment by the insurance company. If your insurance company has not paid your account in full within 45days, the balance is due by you. Please be aware that some, and perhaps all, of the services provided may be non-covered services and not considered reasonable and necessary. All co-pays and deductibles are due at the time of service.
No Insurance
There will be no insurance involvement (Health, Auto or Work Comp). A walk out receipt will be furnished when payment is made for Flex or other Cafeteria Plans. You will assume responsibility for complying with the conditions of the Time of Service (TOS) contract and Doctor’s recommendations without verbal or written reminders from representatives of SCO. You also understand a TOS service is payable at the time services are rendered. In the event it becomes necessary for SCO to send a statement at the end of the month all services will be changed to the regular fee schedule with finance charges, unless other arrangements have been made.
Automobile Accident Insurance
If you have med pay on your auto policy a claim will need to be established with your insurance company as soon as possible. Please contact your insurance company (and attorney if necessary) so that a claim can be quickly established. Please remember that you are still personally responsible for you account but that you will not be required to pay as services are rendered. Remember you are responsible to timely pay your bill regardless of inaction or mis-action by your automobile insurance carrier.
Usual and Customary Rates
Our practice is committed to providing the best treatment for our patients and we charge what is usual and customary for our area. You are responsible for payment regardless of any insurance company's arbitrary determination of usual and customary rates.
Appointments For Minors
The parent, guardian or adult accompanying a minor are responsible for full payment. For unaccompanied minors, non-emergency treatment will be denied unless charges have been pre-authorized to an approved credit card, check or cash.
Missed Appointments
Unless canceled, at least 24 hours in advance, our policy is to charge the rate of a normal office visit. Please help us serve you better by keeping scheduled appointments.
InterestWe reserve the right to charge interest in the amount of 18 % as provided by state law.
