Smith Chiropractic Offices P.C. believes in the value of wellness care for you and your family and have maintained a pricing structure that allows care for all budgets. Our office participates in most insurance programs. We also accept auto accident and workers compensation cases. For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged. Insurance
Your insurance policy is a contract between you and your insurance company. We are not a party to that contract. Please remember that most medical insurance is not designed to pay the true value of the services and may provide for only a portion of the fee. When we or if you call to verify your insurance coverage this is not a guarantee of payment by the insurance company. If your insurance company has not paid your account in full within 45 days, the balance is due by you. Please be aware that some, and perhaps all, of the services provided may be non-covered services and not considered reasonable and necessary under the Medicare Program and/or other medical insurance. All co-pays and deductibles are due at the time of services.
No Insurance
Services rendered with no insurance involvement are reduced to the lowest insurance (Medicare) fee schedule utilized in this office. Services must be paid at time of service. These fees are only for those patient's that have no Insurance or Lawyers involved. If payment can't be made at time of service a credit plan must be approved and signed. If accounts are not paid in full at the end of 30 day period, fees will be changed to Insurances Fees unless credit plan has been arranged.
Automobile Accident Insurance
If you have med pay on your auto policy a claim will need to be established with your insurance company as soon as possible. Please contact your insurance company (and attorney if necessary) so that a claim can be quickly established. Please remember that you are still personally responsible for you account but that you will not be required to pay as services are rendered. Remember you are responsible to timely pay your bill regardless of inaction or mis-action by your automobile insurance carrier.
A “TORT” system is a system where the driver who was at fault is responsible for paying the victim's medical expenses, loss of wages and pain and suffering. If you have a “TORT” policy and are the victim you will probably need an attorney. You must sure the offending party to get your health treatments paid by the opposing party (if you have NO med pay). The insurance company of the driver who causes the accident will be responsible for paying the medical bills after you sue them.
If you caused the accident either by running into another vehicle or property like a brick wall or tree, then your medical payments coverage or “Med Pay” may cover you. We will bill your auto insurance for Med Pay and your group health insurance to try to get your bills paid. You are responsible to timely pay for bills regardless of inaction or mis-action by your automobile insurance carrier or group health insurance carrier.
If you are the victim and have no auto or health insurances to pay your health care bill, we will help you. You will be asked to sign a lien which is a contract allowing our office to be paid after your attorney settles your case against the driver who caused the accident. We will be happy to answer any questions and refer you to a competent personal injury attorney.
Usual and Customary Rates
Our practice is committed to providing the best treatment for our patients and we charge what is usual and customary for our area. You are responsible for payment regardless of any insurance company's arbitrary determination of usual and customary rates.
Adult Patients
Adult patients are responsible for full payment at time of service unless arrangements in writing have been approved.
Appointments For Minors
The parent, guardian or adult accompanying a minor are responsible for full payment. For unaccompanied minors, non-emergency treatment will be denied unless charges have been pre-authorized to an approved credit plan, credit card, check or cash.
Missed Appointments
Unless canceled, at least 24 hours in advance, our policy is the charge ½ the rate if a normal office visit. Please help us serve you better by keeping scheduled appointments.
